Upload Your COVID-19 Vaccine Record 

A message from Tracy Robin, Associate Provost for Student Health Services

Dear Students,

I’m pleased to share that the Student Health Services (SHS) portal is now open and accepting COVID-19 vaccination records and exemption requests from new students and students returning from a leave of absence. Student Health Services will notify new students when they are able to submit measles, mumps, rubella (MMR) and meningitis ACWY documentation.

Please log in to the portal and follow the instructions below to submit your proof of COVID-19 vaccination or an exemption request form. The submission deadline for new students and students returning from a leave of absence has been extended to August 9. Students in countries without access to vaccines should follow the instructions on the Travel Guidance page for completing their quarantine, testing, and vaccination in the U.S. after this date.

How to Submit your COVID-19 Immunization Record

1. Visit shsportal.newschool.edu and log in with your Net ID and password. This is the same log-in information you use to log in to MyNewSchool and your New School email account.

2. Enter your date of birth to confirm your identity and then click the red button that says “Proceed.”

3. Next, click the red button that says “Enter My COVID-19 Vaccine Information.” If you don’t see this button, click “Medical Clearances” in the left sidebar.

4. When the window opens, click the green upload button to add your vaccination record Please note: Only PNG, JPEG, GIF, or PDF files will be accepted. All documentation must be submitted in English or accompanied by a certified translation. Acceptable proof of vaccination includes, but is not limited, to the following: CDC COVID-19 vaccine card, WHO vaccine booklet, or documentation provided by a health care professional/organization/country.

5. A preview of the image or file you selected will pop up. Make sure the photo or document is clear and click “Looks Good.”

6. Next, type in the date(s) on which you received your vaccine(s) and choose the FDA or WHO-authorized vaccine type from the drop-down menu.

7. Finally, click the “Done” button in the bottom right-hand corner of your screen.

Please do not email your record to Student Health Services (SHS). If you previously emailed your vaccine record to SHS, it will not be processed. It will only be reviewed and processed once you have uploaded it to the portal.

You will not receive a confirmation email after your record is submitted or approved. To ensure that your submission has been received, click the “Medical Clearances” link on the left-hand side of the portal screen. If you have successfully uploaded your record, this page will say “Awaiting Review.”

Once your record has been reviewed and approved, the status will change to “Compliant.” You can log in to the portal at any time to check the status of your submission.

How to Submit your COVID-19 Medical or Religious Exemption Request

1. Visit shsportal.newschool.edu and log in with your Net ID and password. This is the same log-in information you use to log in to MyNewSchool and your New School email account.

2. Enter your date of birth to confirm your identity and then click the red button that says “Proceed.”

3. Click the “Downloadable Forms” link in the sidebar on the left.

4. Download and print the form you need.

5. Read through the form carefully and complete it fully.

6. Upload the fully completed form to the portal. Incomplete forms cannot be reviewed or approved.

Please be sure to submit your COVID-19 documentation by August 9 to ensure you receive clearance to access campus facilities for residence hall move-in and/or the start of classes. Student Health Services will notify new students when they are able to submit measles, mumps, rubella (MMR) and meningitis ACWY documentation.
